This page explains what happens to the information you send through the enquiry form on this website.

Who is responsible

Clinical Web Design is run by Dr Husein Essaji. If you have any question about your information, email mail@clinicalweb.design.

Please do not send patient information

The enquiry form is for information about you and your practice only. Please do not include patient details, clinical information or any other special category data. If you send something of that kind by mistake, email me and I will delete it.

What the form collects

The enquiry form asks for:

  • Your name, which is required.
  • Your email address, which is required.
  • Your practice or clinic name, which is optional.
  • Your current website address, which is optional.
  • An optional free text note about what you would like your website to achieve.

Nothing else is requested. There is no analytics, advertising or tracking on this website, and it sets no cookies.

Why it is collected

Your details are used to reply to your enquiry, and where you have asked for one, to prepare and share a sample website for your practice. They are not used for anything else, are never sold, and are not added to a marketing list.

Who handles it

Three service providers process your information on my behalf as part of running the website and my email:

  • Vercel, which hosts this website and runs the code that receives your enquiry.
  • Resend, which delivers the enquiry to my inbox as an email.
  • Zoho Mail, which provides the mailbox where I read and reply to it.

Your rights

Under UK data protection law you have the right to ask for a copy of the information held about you, to have it corrected, to have it deleted, to restrict or object to how it is used, and to ask for it in a portable form.

To make any of these requests, email mail@clinicalweb.design. You will not be charged, and I will respond within one month.

Complaints

If you are unhappy with how your information has been handled, please tell me first so I can put it right. You also have the right to complain to the Information Commissioner’s Office, the UK regulator for data protection, at ico.org.uk.
